New website feature: We are pleased to announce our new patient portal. To access all online patient services, please click on the patient portal tab at the bottom of the menu on the right. If you have given us your e-mail address, you will receive an e-mail inviting you to register for our secure patient portal. If you have not given us your e-mail address, please call the business office at 616-459-3158, option 2, and then option 4. We will update your information and add your e-mail address, which will generate an invitation to you, so you can register for this service. Please note that labs have to be published by your physician for them to be available to you on the portal; this is not an automatic feature. Patient Centered Medical Home-Case Manager
We recently employed a case manager for our Patient Centered Medical Home, Danielle Mysliwiec BSN, RN. Danielle brings with her a passion for improving people's lives, critical thinking skills and patient advocacy. Danielle will be working with patients to increase prevention of diseases and following-up with patients who have recently been discharged from the hospital. Danielle will also work with patients with chronic illnesses to help them better understand and manage their disease. Danielle is excited to improve the health of patients and to be there to answer questions they may have.
